Completing your forms ahead of time helps your first visit go smoothly. Below are the forms most new and current patients need — including intake paperwork (which collects your date of birth and background information), standardized screening questionnaires, and treatment consent — all completed directly on the Solace Center website.

Submissions are stored in a restricted area of our website that is not publicly accessible, and only your name, email, and a reference number are ever included in staff email notifications — never your answers or date of birth. This site does not claim, and is not represented as, a HIPAA-certified data system; if you have concerns about submitting sensitive information online, please call our office instead.
